In-home Services

Non-medical, in-home services are available to help seniors remain living independently in their homes, preventing placement into institutional settings. Caregivers may be supported in their efforts by providing services to individuals they are caring for.

Case Managers on our staff work with the individual and family members to develop a care plan and then meet with them on a regular basis to ensure that their needs are being met. Services are offered by a contracted provider. Needs and eligibility are assessed over the phone or in person. Individuals are then placed on the appropriate waiting list.

Medicaid Wavier

  • 65 years of age or older
  • Frailty level that meets skilled nursing facility admission guidelines
  • Medicaid eligible

Alternatives (also known as HCBA)

  • 18 years of age or older (preference is given to persons 60+)
  • At risk of nursing home placement
  • Meet Federal Poverty Guidelines and have limited assets

Caregiver Support

  • Assists caregivers in their care of individuals 60 years of age and older who are unable to perform routine activities to care for themselves
  • No income or asset limitations

New Choices Waiver

Mountainland is one of several agencies that provides case managements for the New Choices Waiver program (NCW). NCW is overseen and administered by the State of Utah.
