REACH is Resources Enhancing Alzheimer's Caregiver's Health. It is an in-home program, tailored to support caregivers who are caring for loved ones with dementia. REACH provides education, a focus on safety for the patient, support for the caregiver, and skills building to help caregivers manage difficult patient behaviors and decrease their own stress. 
A caregiver education specialist meets face-to-face with the primary caregivers over the course of 6 months and tailors the program to meet individual concerns and needs. Through out the course of meetings we cover:

  • Stress management
  • Problem solving
  • Dementia education
  • Self-care obstacles
  • Dealing with troubling dementia behaviors
  • Community resources
